To serve your health needs, LOOKINCARE collects information directly from you on our website, or orally, or using offline forms, or by e-mails. If you provide us with express, written permission, or an authorization, we may also get information from third parties such as your health care provider or physician. All information that identifies you is referred to as “personal information” and we may collect a variety of personal information, such as your name, residential address, country of citizenship, phone number, email address, contact preferences, age, birth of date, social security number, citizenship ID, passport ID, medical report and credit card information.
Personal information is shared with our network of clinics and health centers to provide you with a health service solution. Your personally identifiable information will be used only as necessary to provide the services that you request, or as otherwise required by law.
Collection and Use of Non-Personal Information
We also collect data in a form that does not, on its own, permit direct association with any specific individual. We may collect, use, transfer, and disclose non-personal information for any purpose.
If we do combine non-personal information with personal information the combined information will be treated as personal information for as long as it remains combined.
